Cannabidiol (CBD) has experienced exponential growth in recent years, establishing itself as one of the most researched cannabis compounds due to its therapeutic potential without psychoactive effects. In 2025, science continues to explore its medicinal applications while debunking many of the exaggerated claims that have surrounded this cannabinoid.
What is CBD and how does it differ from THC?
CBD is the second most abundant cannabinoid in cannabis after THC (tetrahydrocannabinol). The fundamental difference is that CBD is not psychoactive, meaning it does not get you high. This characteristic makes it an attractive option for people seeking therapeutic benefits without altering their state of consciousness.
In Spain, the cultivation of hemp (Cannabis sativa) is permitted provided it contains less than 0.3% THC, a limit established by the European Union since 2023. This legal framework has allowed the development of a CBD industry with 271 companies generating 2,298 direct jobs.
Therapeutic applications with scientific evidence
Refractory childhood epilepsy
The most solid and proven application of CBD is in the treatment of severe forms of childhood epilepsy. The drug Epidyolex (pharmaceutical cannabidiol) was approved by regulatory agencies in the United States (FDA), Europe (EMEA), and Spain (AEMPS) for the treatment of Lennox-Gastaut syndrome and Dravet syndrome.
This approval represents a historic milestone, being the first cannabis-derived medicine to receive full authorization for its proven efficacy in rigorous clinical trials.
Reduction of chronic pain
There is ample evidence of CBD's benefits in managing chronic pain in rheumatic diseases, osteoarthritis, rheumatoid arthritis, and fibromyalgia. Clinical studies show that CBD acts as an analgesic and muscle relaxant, reducing inflammation and modulating pain perception.
Patients treated with doses of 75 mg (1 ml or 20 drops) of CBD every 8-12 hours for 7-14 days experience a significant decrease in symptoms and an improvement in their quality of life. CBD is particularly effective when conventional analgesics and anti-inflammatories do not provide sufficient relief.
Anxiety reduction
Although evidence is preliminary, studies suggest that CBD may help reduce anxiety without the sedative effects of traditional anxiolytics. Preliminary research indicates it can improve perceived sleep quality, reduce stress, and increase overall life satisfaction.
However, it is crucial to emphasize that in Spain, CBD is not authorized as a medical treatment for anxiety, and any therapeutic use must be supervised by healthcare professionals.

Research published in the journal eNeuro in 2025 demonstrated that inhaled CBD can reduce neuroinflammation in mouse models of Alzheimer's disease. The treatment reduced the expression of key regulators of brain inflammation, correlating with a decrease in pro-inflammatory molecules in the nervous system.
These results are promising but very preliminary, requiring further studies in animals and humans before benefits can be confirmed in Alzheimer's patients. Nevertheless, they reinforce the research pathway based on inflammation as a key factor in neurodegenerative diseases.

CBD has shown efficacy in reducing spasticity and muscle contractions associated with multiple sclerosis and spinal cord injuries. The drug Sativex® (a combination of THC and CBD) has been authorized in Spain for this indication for years.
Benefits without conclusive evidence
Many attributed benefits of CBD lack sufficient scientific support:
"Curative" properties: There is no clinical evidence to state that CBD cures specific diseases such as cancer, diabetes, or cardiovascular diseases.
Anti-aging: Although widely promoted, evidence for anti-aging effects is anecdotal and not supported by rigorous clinical studies.
Weight loss: There are no studies supporting that CBD promotes weight loss, although it can increase appetite in some people.
It is crucial to distinguish between pharmaceutical CBD used in clinical trials and commercial products of variable quality available in stores. The composition, purity, and dosage of the latter can vary significantly, affecting both efficacy and safety.

Prohibition of human consumption: Technically, CBD is not authorized for direct human consumption except in registered medicines like Epidyolex or Sativex®.
Cultivation licenses: To commercially cultivate CBD hemp, a license from the Ministry of Agriculture is required, using EU-authorized varieties with less than 0.3% THC.
The THC content is strictly regulated. Flower tops/buds are considered narcotics even if the variety is ≤0.3% THC, limiting legal use to fiber, grain, or seed.
