In October 2025, Spain approved the royal decree on medicinal cannabis, allowing its use in hospitals for specific conditions such as multiple sclerosis, refractory chronic pain, and severe epilepsy. This regulation marks a historic milestone in Spanish healthcare.
Which patients can access medicinal cannabis?
The AEMPS (Spanish Agency of Medicines) will establish specific monographs for:
• Multiple sclerosis with spasticity
• Refractory chronic pain to other treatments
• Severe epilepsy in specific cases
• Nausea and vomiting due to chemotherapy
Prescription and dispensing process
Hospital specialist doctors will be the only ones authorized to prescribe magistral cannabis formulations. Hospital pharmacies will prepare the products following strict quality and safety protocols.
Differences with other European countries
Unlike other countries such as Germany, Spain limits access exclusively to the hospital setting, excluding primary care and expressly prohibiting cannabis for smoking for recreational purposes.
